The undersigned agrees to rent the apartment located at 5414 Park Ave. West New York NJ 07093

By signing below the tenant acknowledges they received the Consumer Information Statement of New Jersey Real Estate Relationships from the brokerage firm involved in this transaction prior to the first showing of the property.

I, the Tenant, accept and acknowledge that monthly rent of $1,995.00 Furthermore, I accept and acknowledge that the rent is due to the Landlord on the 5th of each month and that payments received after the 5th of each month will be subject to late fees.

I Richard Henao am an authorized representative of Selling New Jersey Realty Group and I am working on this transaction as a transaction agent

All applications are subject to credit investigations and approval by the Landlord.

Authorization to Release Information
I hereby authorize Selling New Jersey Realty Group, LLC and/or their assigned credit bureau to obtain any and all information regarding my employment, checking and/or savings account, credit obligation, rental information, and all other credit matters that they require in connection to lease an apartment. This consent is effective for a period of six months from the date of this consent. This form may be reproduced or photocopied and that shall be as effective as the original which I have signed.

Approval
In the event that Landlord or Selling New Jersey Realty Group, LLC elects not to accept my application for any reason, I agree that I will not assert any claim for damages against Landlord and/or Selling New Jersey Realty Group, LLC.

Deposit
In order to have an apartment held for me, a non-refundable deposit of $2,992.50 must be given to the office. Deposits are only refunded in the event that an application is not approved by management.

Application
I, the tenant agree that the $50.00 application fee is non-refundable.
